The 2021 Study of Culture & Community in a Time of Transformation
Sweepstakes (“Sweepstakes”) of Slover Linett Audience Research (“sponsor”) begins on April 5, 2021 at 12:01 AM and ends on May 31, 2021 at 11:59 PM (“Promotion Period”). The time shall be Central Standard Time, determined by the Sponsor’s timekeeping systems, and the timeliness of any action related to the Sweepstakes will be determined at the sole discretion of the Sponsor.

 

 

                                                                                                                                                                      Open only to legal residents of the United States and Canada who are 18 years of age or older as of date of entry. Void where prohibited by law. Individuals who are employees of Sponsor or employees of participating cultural organizations, or individuals who are immediate family members (spouse, mother, father, in-laws, grandmother, grandfather, brother, sister, children and grandchildren) and/or those living in the same household of such employees, are not eligible to participate in the Sweepstakes. By participating, entrants agree to be bound by the Official Rules and the decisions of Sponsor.

 

 

 

TO ENTER: Once during the Promotion Period, complete a survey on behalf of a cultural organization participating in the 2021 Study of Culture & Community in a Time of Transformation, and follow the on-screen instructions to submit survey responses for a chance to win a prize (“Prize”). Entries must be received during the Promotion Period to be eligible to win a Prize. All information you provide must be truthful, accurate and complete. Upon successfully submitting your entry as described above you will receive one (1) entry in the Sweepstakes. Limit one (1) entry per person during the Promotion Period. Illegible and incomplete entries are void. Sponsor is not responsible for lost, late, misdirected entries or entries not received. In the event of a dispute over who submitted an online entry, the entry will be deemed to have been submitted by the authorized account holder of the e-mail account submitted during the survey. The authorized account holder is deemed to be the natural person who is assigned to an e-mail address by an Internet access provider, service provider, or online organization responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address.

 

 

                                                                                                                                                            WINNER SELECTION: Winners will be randomly selected on or about July 31, 2021 from all eligible entries received during the Promotion Period.  Entries will carry over until all winners have been selected and prizes distributed.  Decisions of Sponsor are final on all matters. Limit one prize per person. Odds of winning will depend upon the number of eligible entries received.

 

 

                                                                                                                                                              PRIZE:  Twenty-five (25) prizes will be awarded. Winners will each receive a $100 USD VISA gift card distributed by email. No substitution or transfer/assignment of prize by winners permitted. Sponsor reserves the right to substitute any prize with a prize of equal or greater value in its sole discretion. Subject to all terms, conditions and restrictions on face of tickets, gift cards, certificate, vouchers or imposed by issuer. Under no circumstances will more than the advertised number of prizes be awarded. Prizes are awarded “AS IS.” Prize winner will be emailed the gift card code to the email address provided in the survey.   Failure to redeem a prize within one (1) year will void the winner’s right to the prize and an alternate winner will be chosen as determined by random draw from the remaining eligible entries.

 

                                                                                           

Federal, state/provincial, and local taxes on prizes, if any, and any other costs, fees and expenses not listed above as specifically included as part of the prize are the sole responsibility of winner. Tax-related forms and documents may be issued by Sponsor if required by law.

 

 

 

WINNER NOTIFICATION AND RELEASES: On or around the time of the drawing date, Sponsor shall notify the potential winner(s) and send the prize. The potential prize winner(s) will be contacted with the information provided at the end of the survey.  Each potential winner may be required, in Sponsor’s sole discretion, to provide a scan or photo of legal ID proving date of birth and legal residence; failure to do so will result in winner being deemed ineligible for receipt of prize.

 

 

                                                                                                                                                            LIABILITY RELEASE & INDEMNIFICATION: By entering this Sweepstakes, you agree to release, discharge and hold harmless Sponsor, officers, employees, agents and assigns (the “Released Parties”) from any claims, losses, and damages of any kind arising out of, or relating to, your participation in the Sweepstakes or any Sweepstakes-related activities and the acceptance and use, misuse, or possession of any prize awarded here under. The foregoing includes, without limitation, any claim for personal injury, property loss or damage, or death arising in any way in connection with the Sweepstakes.

 

 

                                                                                                                                                              OTHER TERMS: Sponsor is not responsible for any typographical or other error in the printing, the offering or the announcement of any prize or in the administration of the Sweepstakes. Sponsor is not responsible for late, lost, damaged, incomplete, illegible, faulty, or incorrect transmissions, incorrect announcements of any kind, technical hardware or software failures of any kind, any damage to any person’s computer related to or resulting from participating in the online portion of the Sweepstakes, lost or unavailable network connections, or failed, incomplete, garbled or delayed computer transmission that may limit a user’s ability to participate in the Sweepstakes. Sponsor reserves the right to cancel or modify the Sweepstakes or any portion of it for any reason. Sponsor reserves the right to disqualify, freeze, or prohibit the participation of an individual if fraud or tampering is suspected, or if the individual fails to comply with any requirement of participation or with any provision in these Official Rules.

 

 

                                                                                                                                                 

GOVERNING LAW & JURISDICTION: Except where prohibited, entrants agree that any and all disputes, claims, and causes of action arising out of or connected with the Sweepstakes shall be governed by the laws of the State of Illinois and resolved exclusively in a court of competent jurisdiction located in Chicago, Illinois.  Any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred by entrant with regard to the Sweepstakes, but in no event attorney’s fees, punitive, incidental, consequential or other damages.

 

 

                                                                                                                                                SEVERABILITY: If any provision(s) of these Official Rules are held to be invalid or unenforceable, all remaining provisions hereof will remain in full force and effect.
